Please note that all camper forms are now online and can be completed by access through your account.

The following forms are required for each enrolled camper: (1) Health Form (4 pages); (2) Transportation Form; and (3) Summer Questionnaire. In addition, we request a teacher reference form for each new camper. To access these forms, please click here
